The Community Nursing Teams provide nursing care, health education and support for patients who are unable to leave their home due to illness, or who have been identified as requiring care from the community nursing service. Community Nursing is not provided by the practice but by Shropshire Community Trust.
Services

Travel Health
If you require NHS travel vaccinations, we are happy to provide these. However, we are unable to give travel advice.
If you are travelling please attend a private travel clinic (available at some pharmacies) and take with you a copy of your vaccination history. We can provide a list of your previous vaccinations or this can be found on your NHS App. If you then require NHS vaccinations we ask that you provide documentation from the travel clinic you have attended, we need to have this two weeks before any vaccines can be administered. The nursing team need to review the request and order in the required vaccines. Please book an appointment with the practice nurse two weeks after providing your travel clinic documentation.
Free of charge NHS vaccinations commonly required are
- Polio (given as a combined diptheria/tetanus/polio vaccine)
- Hepatitis A
- Typhoid
- Cholera
Other vaccinations need to be given at the travel clinic and maybe chargeable.
It is important to think about travel health well before your trip we suggest three months.
Travel Clinics are not only about vaccines but also offer advice on pre-travel risk assessment, country specific information and advice, vaccinations and malaria prophylaxis.
If you require anti-malaria tablets, we can no longer issue a prescription for this.
It is your responsibility to ascertain which vaccinations/anti-malaria tablets you need and also to inform yourself fully of recommended travel safe precautions.

Young People
Albrighton Medical Practice welcomes all young people.
You can come on your own or choose who comes to the appointment with you; this may be a parent, carer or a friend. If you would prefer to see a male or female doctor/nurse, we will do our best to arrange this. You can make your own appointment, even if you are under 16.
The Medical Practice provides a confidential service to all patients including under 16’s. The only reason why we might have to consider passing on confidential information without your permission, would be to protect you or someone else from serious harm. We would always try to discuss this with you first.

We want to get better at communicating with our patients. We want to make sure you can read and understand the information we send you. If you find it hard to read our letters or if you need someone to support you at appointments, please let us know.
